Project Homeless Connect is a City of San José effort to assist the chronically homeless population on the road to self-sufficiency by providing a concentration of medical, mental health, housing information, financial assistance, veterans services and an array of other services.

In addition to connecting with services, PHC 5 is the place to get hard-to-come-by necessities: hair cuts, bus tokens, bathroom tokens, gift bags containing socks, personal hygiene kits, and Community Voice Mail numbers
