Please join the City of San Jose and the County of Santa Clara for the fifth Project Homeless Connect (PHC) on Wednesday, December 5th from 9:00 AM. to 3:00 PM. at the San José McEnery Convention Center.

PHC is a one-day concentrated effort to assist the City’s chronically unhoused residents on the road to permanent housing and self-sufficiency by connecting them to a range of medical, housing and legal information and other services.
