More than a third of the nearly 8,000 people interviewed for the 2004 Santa Clara County Homeless Census are chronically homeless – defined for these purposes as those who have been homeless for more than a year or have had four or more homeless episodes in the last three years and are disabled with a mental health condition, physical disability or substance abuse problem.  As an integral part of our response to homelessness, and to ensure continued federal funds, our community embarked on a thorough investigation into solutions that will end chronic homelessness in Santa Clara County by 2015. That blueprint – Keys to Housing – follows.
